§ 42-92. Application generally.
Before any person may be employed in any occupation or establishment licensed or permitted under the provisions of this chapter, such person shall first make application for a permit therefor from the supervisor of liquor control in conformance with the provisions of this section and this chapter. All such applicants shall fill out a form furnished by the bureau of liquor control. Such application shall contain but not be limited to the following:
(1)
The name, residence address and residence telephone number of the applicant, all names, nicknames and aliases by which the applicant is known, and the two previous addresses of the applicant immediately prior to the last address.
(2)
The title or type of employment for which the permit is sought.
(3)
The name, address and all telephone numbers of the establishment wherein the applicant is or is to be employed.
(4)
Social security number, driver's license number and state of issuance, and date of birth.
(5)
The applicant's weight, height, color of hair and eyes, and sex.
(6)
Satisfactory proof that the applicant is over 18 years of age.
(7)
A complete statement of all convictions of the applicant for any felony or misdemeanor or ordinance violation, except minor traffic violations.
(8)
A complete set of the applicant's fingerprints.
(9)
Portrait front-face photographs of the applicant, of appropriate size.
(10)
A statement signed by a licensed physician certifying that the applicant was examined and found to be free of communicable disease on a date within the last 30 days.
(11)
Such other information, identification and physical examination deemed necessary by the supervisor of liquor control to discover the truth of the matters required to be set forth in the application.
(12)
Authorization by the applicant for the city and its agents and employees to seek information and conduct an investigation into the truth of the matters required to be set forth in the application.
(13)
Written declaration by the applicant that the information required by this section as set forth in the application is true and correct to the best belief and knowledge of the applicant. Any deliberate falsification of any material fact or statement contained in the application shall be grounds, in the discretion of the supervisor of liquor control, to deny such application.
