§ 34-222. Fee for copies of records generally; corrections and amendments to records.

  • Upon the payment of a fee of $3.00 for each certificate, the director of health shall furnish a copy of the official birth or death record on file in his office. Upon receipt of proof of marriage by parents or a certified copy of a court order or decree of adoption, he shall prepare a new certificate in the new name of the legitimated or adopted person for a fee of $3.00 for each certificate. The evidence upon which the new certificates are made and the original certificates shall be sealed and filed and may be opened only upon order of the court. A charge of $3.00 will be made for all corrections and amendments to vital records. To search the files for a birth or death record for family history or genealogical purposes, a fee of $3.00 for each record shall be charged.
