No person shall engage in, carry on or conduct the operation or maintenance of any
driver training school within the limits of the city without first obtaining a license
therefor from the commissioner of revenue. Applications for licenses shall be made
to the commissioner, on forms provided by him, setting forth the name of the applicant;
and, if an individual, his place of residence; and, if a firm, the names and places
of residence of all of its members; and, if a corporation, the names and places of
residence of all of its officers, the state of its incorporation, the name under which
the driver training school is to be conducted, and the location of its principal place
of business.
