§ 10-264. Lost licenses.

(a)

Whenever a license issued under this chapter is lost or destroyed, a duplicate in lieu thereof may be issued by the director to the licensee.

(b)

Application for a duplicate license shall be by affidavit of the licensee, filed with the director, request that a duplicate license be issued, and set forth:

(1)

The approximate date upon which the license was lost or destroyed.

(2)

The circumstances under which the license was lost or destroyed.
